- Amendments to Bankruptcy Official Forms Effective
October 1, 2006 (updated 09/25/2006)
At it's September 19, 2006 session, the Judicial Conference of the United
States approved the proposed amendments to Bankruptcy Official Forms
1, 5, 6, 9, 22A, 22C, and 23 and a new Exhibit D to Official Form 1
to take effect on October 1, 2006. The forms are available at http://www.uscourts.gov/bankform/index.html

- Instructions for E-Filers regarding Exhibit
D: Since Exhibit D is an exhibit to the petition, please file
the exhibit as part of the petition (during case opening). The Credit
Counseling Certificate should continue to be filed separately, immediately
